Gift Aid Declaration
Gift Aid is reclaimed by the synagogue from the tax you pay for the current tax year. Your address is needed to identify you as a current UK taxpayer.
In order to Gift Aid your donation you must tick the box below.
Please Gift Aid my donation
I want to Gift Aid all donations I make in the future or have made in the past 4 years to Southgate Progressive Synagogue. I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year, it is my responsibility to pay any difference.

YOUR NAME (Please print)
Please print your name in the box.
Signature
We need a signature, so please print sign and return this Gift Aid section of the form (between the lines) or the whole form if easier and return it to the office along with the completed direct debit form.
Your email address
Please print your email address
You must notify the charity if you want to cancel this declaration, change your name or home address, or no longer pay sufficient tax on your income and/or capital gains. If you pay income tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your Gift Aid donations on your Self-Assessment tax return or ask HM Revenue and Customs to adjust your tax code.
If you cannot complete the Gift Aid declaration please indicate this below instead.
I regret that I/we cannot sign a Gift Aid declaration because I do not pay any tax on income, a pension, building society interest or any other investments. (Note - you probably don't complete a self-assessment tax return in any case; for most people income tax in deducted at source from your employment, pension or investment income.)
I cannot sign the Gift Aid declaration.
